By participating in the Navina Provider Challenge (the "Competition") at Value-Based Care Across the Continuum (the "Conference"), you agree to the following terms and conditions:
Eligibility:
- The Competition is open to all registered attendees of the Conference.
- Participants must be at least 18 years of age or older to enter.
Entry Requirements:
- To enter the Competition, you must complete the official entry form on the Navina landing page.
- Entry forms must be submitted between October 11, 2023 and October 22, 2023 (the last day of the Conference).
- Only one entry per person is allowed. Multiple entries will result in disqualification.
Prize:
- The prize for the Competition is an Apple watch and is subject to change without notice.
- The prize is non-transferable and may not be redeemed for cash or any other substitution.
Winner Selection:
- The winner will be selected based on having the highest usage rate during the four-week measurement period, beginning October 23, 2023 and ending November 20, 2023. The usage rate will be calculated based on the number of Navina-generated “Patient Portraits” with HCC recommendations reviewed by the provider or any Navina user within SC House Calls.
- The winner will be announced up to 30 days after the Conference via email.
Release of Liability:
- By participating in the Competition, you release and discharge Navina Technologies Ltd. its affiliates, subsidiaries and employees from any and all liability, claims, actions, or damages arising out of or in connection with your participation in the Competition or acceptance of the prize.
Publicity and Use of Information:
- By entering the Competition, you grant Navina permission to use your name, likeness, photograph, and any other personal information for promotional and marketing purposes without further compensation, unless prohibited by law.
Changes and Termination:
- Navina reserves the right to modify, suspend, or terminate the Competition at any time, for any reason, without notice.
Governing Law:
- This Competition is governed by and construed in accordance with the laws of [State/Country] without regard to its conflict of law principles.
